  • Wednesdays 10:00 - 12:30 April 13th 2016 - May 18th 2016
  • There is no charge for the workshop
  • 6 session, one day per week, 2 1/2 hour plus one hour for lunch and cooking lesson
  • For participants with Diabetes or Pre-Diabetes
  • Learn new tools to take day-to-day responsibility for your care
  • Increase skills necessary to manage Diabetes
  • Learn how to work effectively with your health care providers

The Chronic Disease Self-Management Program was developed by the Stanford University Patient Education Research Center and has found that people who take the program have improved their healthful behaviors and their health status.
